Pennsylvania has a decennial report that is due every ten years in the years ending with the numeral "1". Meaning the most recent decennial report is due sometime within Jan 1, 2011 and Dec 31, 2011, for the 1-1-2002 to 12-31-2011 period. The only way to file is to send in the PDF form, "Decennial report of association continued existence."
