  • Filing fee: $50 Secretary of State fee.
  • Recording fee (approximately $30); each Alabama Probate Judge has a different fee schedule, so you’ll need to contact the Probate Judge’s office in the county where your AL corporation was formed for the appropriate fee.
  • Domestic Corporation Amendment to Formation/Articles Form (can be found on the Alabama Secretary of State’s website).
  • The filer’s original signature.
  • Alabama Entity ID Number (you can find this six-digit number by performing an Alabama name search)

Provide an original and two copies of Domestic Business Corporation Amendment to Formation/Articles form, to the Judge of Probate in the county where the original Certificate of Formation is filed, by mail or in person, with both the filing fee and recording fee. An original signature is required.

No, Alabama does not furnish a form for restated Certificate of Formation.
