  • pay overdue penalties or fees for five months

The DC Reinstatement of Domestic Filing Entity (Form GN-5) is a one-page document that requires basic information about your DC LLC. You’ll need to provide:

  • your entity name
  • an alternative name, if your old name has been adopted by another company
  • the date your LLC was administratively dissolved
  • the date of filing

The form also includes a statement that your LLC has appointed a registered agent and is current on biennial reports and fees. It must be signed by a manager, member, or authorized person.

How do I file the DC Reinstatement of Domestic Filing Entity?

You can submit the DC Reinstatement of Domestic Filing entity form by mail, in person, or online. Payment options depend on how you file.

  • Mail: Check or money order made payable to “DC Treasurer”
  • In Person: Cash, credit card, check, or money order
