You may also download the Limited Liability Company Reinstatement form, fill it in completely and submit it to the Division of Corporations by mail or in person, along with all necessary fees.

  • Past due annual report fees ($138.75 per each year since dissolution)
  • Limited Liability Company Reinstatement form (if filing by mail)
